Why did you choose this breed?
Tell us a little about yourself, including your legal name, address and contact information. Please upload a photo of yourself as well as the living environment where the puppy will be raised. (living room/kitchen and backyard/green space)
Number of children and ages, if any?
How many hours a day will the puppy/dog be left alone?
Do you have any objections to a spay/neuter contract?
Who will be the primary caretaker of the puppy?
Do you currently have any pets? If so, please provide the name and contact information of your veterinarian. We ask you contact the clinic granting us permission to speak with them on your behalf.
Do you have a gender preference for your puppy? (Any, Female, Male)
Do you have a color preference for your puppy? (sable, red & white, or tri color)
Lastly, please list the names and contact information for 3 character references from people who have known you for at least 2 years. Ask permission to use them as a reference before submitting their information and make them aware we will be reaching out.
